There are two culprits for why non-healing ulcers may form on your extremities; they are typically caused by a traumatic wound or prolonged and sustained pressure. The wound is then unable to heal properly because of underlying venous or arterial issues. Most wounds are venous stasis ulcers, or caused by underlying venous insufficiency. Arterial ulcers, or ischemic ulcers, are caused by limited blood supply to the extremities due to atherosclerotic buildup in the arteries. What are the symptoms of a venous ulcer? Venous ulcers form from untreated venous insufficiency. These types of wounds tend to be irregular in appearance. Venous ulcers may begin small in size. If left untreated, they could grow to be massive and deep, sometimes penetrating all the way to the bone. While venous ulcers can form from a trauma to the skin, they also form from excessive swelling to the extremity. For this reason, the wound will often ooze a pus or clear liquid. They are typically very painful and tend to be located on an area of the skin where the bone is close to the surface, like the ankle or side of the foot. What treatments are available for venous ulcers? There is an abrupt transition from healthy skin to wound. The skin surrounding the ulcer may also be reddish and shiny in appearance. The skin will also feel cold to the touch. Arterial ulcers may end up leading to gangrene, or tissue death. The skin will appear very dry and the area will be purple or black. What treatments are there for arterial ulcers? To heal an arterial ulcer, blood flow must be restored to the ischemic area. Typically, the only way to restore this blood flow, once the symptoms have progressed to the point of ulceration, is through surgery. There are many types of surgery available. Typically, your surgeon will begin by performing an angiogram. This invasive, diagnostic test will help them determine the site of the blockage. They will then decide whether to perform an atherectomy, angioplasty, or to place a stent. Atherectomy is when the surgeon uses a catheter with a blade on the tip to extract the plaque build up from inside the artery. An angioplasty is a procedure where the surgeon uses a catheter with a balloon tip to expand the walls of the artery to restore blood flow. Sometimes, neither of these procedures reopen the vessel sufficiently. In this case, the surgeon would then place a stent, a small wire mesh tube, to permanently hold the arterial wall open. At Milford Vascular Institute, we perform these procedures in our office at our Outpatient Based Laboratory (OBL) every week. Our OBL offers the state of the art technology and procedures of a leading hospital, with the comfort and convenience of a quick trip to the doctor. If the disease continues to progress, your surgeon may then decide to perform a bypass surgery. During these procedures, surgeons create an alternate route for blood flow to avoid the area of blockage and restore direct flow to the peripheral extremity.These are major surgeries that require general anesthesia and at least one overnight stay in the hospital. These unsightly and often painful veins most commonly occur in your legs. What causes varicose veins? Varicose veins are typically a result of increased venous pressure. Patients can experience an increase in venous pressure for a variety of reasons. The main reason we see and treat for at Milford Vascular is caused by a disease called venous insufficiency . Venous pressure can also increase due to a decrease in cardiac output, renal failure, or arterial dilation which leads to venous obstruction. Chronic venous insufficiency is a disease which affects 60% of the American population. The vein's job is to bring blood back to the heart. Leg veins face an uphill battle - they are fighting against gravity. The way veins counteract this effect is by (203)882-VEIN having one-way valves in them. The valves close as the blood passes towards the heart and they do not allow blood to flow back towards the feet. In patients with venous insufficiency, these valves wear out over time. Laser vein treatments Milford Vascular Institute specializes in the treatment of varicose veins using laser vein procedures. In this procedure, laser energy is used to close the vein. In the office, the leg is cleansed and prepped with Betadine solution. Lidocaine is used as a local anesthetic. The abnormal vein is identified with ultrasound and punctured with a small needle. The laser catheter is inserted into the vein and advanced under ultrasound guidance to the abnormal valve. The vein is anesthetized with a lidocaine and saline solution. The laser is activated and cauterizes the vein from the valve to the mid-thigh or knee. The catheter is then removed and a compression stocking is placed for 10-14 days to allow the vein to completely close. Follow-up ultrasound confirms successful closure. In time, the body will absorb the vein and it will appear on ultrasound as if the vein has been removed. Venogram Milford Vascular Institute uses venograms to diagnose vascular disease of the veins, like non-thrombotic iliac vein lesions (NIVL). What is a venogram? A Venogram is a procedure in which an x-ray of the veins is taken after a special dye is injected into the veins. Why do I need a venogram? A venogram is commonly used to assess the status of a vein or system of veins, find blood clots within the veins, assess varicose veins before surgery, find a vein in good condition to use for a bypass procedure or dialysis access, help a physician place an IV or a medical device, such as a stent, in a vein, or guide treatment of diseased veins. What can I expect when having a venogram? Venograms are outpatient procedures, meaning they do not require an overnight stay in the hospital. When you come in for the procedure, you will be greeted by one of our friendly nurses. She will help you dress and prepare you for the procedure. Then, the CRNA will come in and meet with you to go over the medications you will be receiving and their side effects. Finally, the doctor will come in and discuss the procedure with you one last time. You will then be brought into the operating room. Our medical assistant will begin to prepare you for the procedure and simultaneously, our CRNA will begin to administer the anesthesia. Venograms are performed under twilight sedation. You should expect to have amnesia after the procedure; you will have no memory of it even happening. After the procedure, we will move you to a recovery room. You will stay in the recovery room for at least 2 hours to allow for the anesthesia to fully wear off. Once our nurse has determined you are fully recovered, you will be able to walk out of the building with minimal limitations. What to expect after a venogram? You may experience some soreness after the procedure. If the provider had to place a stent, you may experience low back or pelvic discomfort for several days. NO SHOW POLICY Milford Vascular Institute No Show/Cancellation Policy for Scheduled Appointments Effective 10/17/2022 As a courtesy, all of our scheduled appointments are confirmed via text message, email, and phone call to the best of our ability at least 24 hours prior to the patient’s scheduled time. We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. In these circumstances it is vital you notify our office as soon as possible and before your scheduled appointment time. FREQUENTLY ASKED QUESTIONS Questions about Varicose Veins? Milford Vascular Institute has the answers. Q: Will this procedure hurt? A: Before the procedure begins, the provider will fully numb your leg with a local anesthetic. This is the only thing you should feel during your procedure; the small sting of the needle injecting the anesthetic. Afterwards, you may feel sore. You can take ibuprofen, Tylenol, Aleve or other NSAIDs to help counteract this feeling. Q: What are my limitations after my procedure? A: After your procedure you should walk as much as possible. You must wear a compression stocking during waking hours for 10-14 days after your procedure. Avoid sitting with your leg bent for any period of time longer than 20 minutes. You can return to normal activities including the gym after 3 days. Avoid any heavy lifting for 7 days after your procedures. Q: Can I fly after my procedure? A: You should not fly for at least 2 weeks after your procedure. You should not fly for at least 3 days after you have injections. If you must fly any time during the 6 months following the procedure you should wear your compression stockings on the trip. Knee high stockings are acceptable. Q: How long do I need to wear compression stockings after my procedure? A: You need to wear compression stockings for 2 weeks after your procedure. After injections, you need to wear compression stockings for 3 days. Q: Can I go in A pool or hot tub? Can I soak in the bath? A: You need to wait 3 days before going swimming or soaking in the bath tub. Q: Can I eat before my procedure? A: Yes, we encourage you eat something before your procedure. This is especially true if you plan to take valium for the procedure. The valium may make your stomach uneasy. Q: How long will the procedure take? A: The procedure itself is very quick. It should not take more than a few minutes. However, you should plan on being in the office for about an hour to an hour and a half. Q: When can I workout? A: You should wait three days before going for a run. You should avoid heavy lifting (nothing more than 10 lbs) for 7 days after your procedure. Q: I have a pulling sensation in my leg after my procedure. Do I need to be concerned? What should I do? A: The pulling sensation is from the vein closing. The vein will turn into a chord and you may feel it on the inside of your leg.It is completely normal. It will go away after a few days with the compression stockings. You can use a warm compress or gentle massage to help take away some of the discomfort. Q: Where can I purchase compression stockings? A: Milford Vascular Institute offers a full stock of the most common sizes for compression stockings. These stockings are available to purchase at MVI as a convenience for our patients. We are unable to process stocking purchases through your insurance. MVI is unable to sell stockings to Medicaid patients. Medicaid patients receive 2 pairs of stockings at no cost to the patient every year. These stockings must be obtained from a surgical pharmacy. Surgical pharmacies are also able to order custom fit stockings in the event MVI does not have a proper fit for you. Q: Can I wash my stockings? A: Yes, you can machine wash your stockings on normal settings. Do not use a dryer, you must air dry your stockings. Q: Will I need a ride for my procedure? A: You only need to arrange for a ride if you plan on taking Valium (Diazepam). Nothing done during the procedure will inhibit your ability to drive. Varithena® (polidocanol injectable foam) is a prescription medicine used to treat varicose veins caused by problems with the greater saphenous vein (GSV) and other related veins in the leg's GSV system. Varithena® improves symptoms related to or caused by varicose veins. What to expect during the treatment: Treatment with Varithena® does not require any incisions or general anesthesia. It usually takes the provider less than an hour to administer Varithena®. Varithena® is administered by a provider into the malfunctioning veins. Varithena® is administered through a catheter or by direct injection into the vein. The foam fills the desired section of the vein, and the diseased vein collapses. Blood flow shifts to healthier veins nearby. The doctor usually numbs the injection site, but no additional anesthesia is required.
