When you come in for your initial consultation we ask that you please bring your drivers license or some other form of photo ID, your insurance cards, a list of your current medications, and the completed new patient paperwork. If your insurance requires a referral to see a specialist, you are responsible for contacting your primary care physician to obtain that referral.

Milford Vascular Institute is in network with all major insurance companies. At the time of your initial consultation, you will be responsible for your copayment, if applicable.
